Telehealth evals and processes can be conducted via telehealth across the entire U.S. and all U.S. territories, no need to come to the office, virtual evaluations available. I can provide timely appointments and quality care when a substance abuse evaluation is required or recommended. Call me and take the first step in the process to return to work safely. I am properly credentialed and qualified to perform a DOT, NASAP, ASAP and DISA SAP evaluations and return to duty processes. $325 for Initial Evaluation, $325 for Final Evaluation, HSA and FSA cards and insurance accepted for appointment cost payment. Costs of recommended education and treatment incurred by employee in conjunction with evaluation fees. Insurance, HSA, and FSA cards can be utilized for these costs. Costs of required drug/alcohol testing incurred by employee in conjunction with evaluation fees can use HSA/FSA to pay. Additional services include case management, resource referral, on site organizational trainings, and management and organizational consultation services available as well, contact for pricing.
